Ground Rules

Codex may write documents in this folder when the human asks or when a useful review artifact should be preserved.
Codex should not use this folder to hide decisions from maintainers; important project decisions still belong in normal docs, ADRs, issues, or PR discussion.
Codex does not push, merge, approve, or make live operational changes.
Codex comments are advisory. The human maintainer is the final authority.
Codex should stay scoped to the active task, PR, commit, or design question.
Codex should prefer concise, actionable notes over broad speculation.
