Merge upstream/dev into mqtt-bridge-implementation

Bring in 267 upstream commits (v1.16.0, kiss_modem targets, build fixes,
docs updates) while preserving MQTT observer build targets and the
GC1109 FEM implementation for Heltec Tracker V2.
