Subject: [PATCH] Fix README video: restore HD thumbnail, add YouTube caption

Switch back to maxresdefault.jpg (1280x720) from hqdefault.jpg
(480x360 with black letterbox bars). Add small caption below the
thumbnail so visitors know it's a clickable video.
