MANI K 5e78aad7d7 Default request token errors to privacy-preserving responses (#20243 )

## Summary

- default `request_token_inhibit_3pid_errors` to `true` to prevent
account enumeration
- document the new default and the explicit opt-out
- add an upgrade note and regression coverage for the default

Fixes #18415.
