Erik JohnstonandClaude Opus 5.5 4d4233091f Fix review findings on sliding sync's query cuts ...

- Run the self-leave query again on every request. A leave committed
  before the membership stream cache heard of it would have dropped a
  room the user was in at the `to_token`, because the snapshot read
  leaves self-leaves out.
- Re-read `instance_map` only when a stored room token names a writer
  the cached table lacks, not on any failure to load the stored state.
- A writer missing from the map while formatting `prev_batch` drops the
  cached table, so the next request reads it again.
- On a room's first send with heroes, reuse the combined state read and
  read only the heroes' memberships it did not cover, where the whole
  required state was read a second time.
