* GitHub issue templates: : Remove leading "[Problem]" in the title and the requirements to fill out all fields (#10779)

* remove leading "[Problem] " in the title

We already set a tag for this

* remove the need to enter all field in a bug report

there are certain cases where this isn't needed and filling them out with blank texts just to remove the text with an edit after filing feels dumb.
